Jacob was born 02/14/1992 he was 32 years old and admitted to Trident Medical Center on April 1st, 2023. A doctor determined that he should be discharged on April the 2nd, 2023. The attending Doctor was Doctor Averette, who has been released by Trident and we have been unable to find him anywhere. We would like to ask him some questions regarding Jacob's care and the Doctor's actions.
Following are images in date order taken by his mother Celina Wallace. Jacob started his stay on the fourth floor and then was moved to the third floor, then the second floor. His blood work came back bad, it was evident that Jacob had an infection - that the doctor seemed to refuse to treat. Jacob had a rapid heart rate and high blood pressure, all displayed on the images below.
With a high white blood cell count, high blood pressure and heart rate, Mrs. Wallace threatened to sue if he was not treated immediately. Jacob was dropped on the floor and left there for four hours. His bladder was full, lying in urine which resulted in bedsores as you can see by the images below.
Finally a catheter was applied, the image shows blood in his urine, yet another sign of infection, and signs of distress being untreated. Proven by the fact that his WBC was lower when he arrived and 3 days latter increased. Take a look at the seventh image down. Please note the following images and copy below.
As you can see by the six images above, Jacob's WBC was high, yet on the screen you can see that Trident had scheduled his discharge for the next day.
Here you can see where the urine clearly display blood and the screen showed rapid heart beat and high blood pressure, even on the 7th of April 2023
Jacob fell out the bed and you can see the burses on his leg and below, his ankle.
The burses where there on the 18th of April, and still on the 24 of April. Above the image on the 26th of April showed sever bedsores basically left untreated.
The video on the left was taken on April the 5th 2023. The video on the right was taken on April 26th, 2023 and shows Jecob certainly in distress. Trident had order his discharge the same day that the video on the right was taken. How could this possible be considered humane care. Jacob's new doctor was Dr. Simmons who did treat some Jacobs symptoms, explained that Trident was demanding that he been discharged from the hospital.
Jacob was discharged on 2 days after the video on the right, April the 28th. Jacob never really recovered from the abuse, miss diagnosis, demands for discharge, negligence in treating his conditions. His vitals were never right after his discharge and Jacob succumbed to condition and passed on October 22, 2024.
We contend that if Trident had properly treated Jacob's condition, showed compassion and did what they were suppose to do, Jacob would be alive today. His body just could not ever recover from his stay at Trident.

Jacobs's DOB 2-14-92 = 32 yrs old.
Went into the hospital on April 1, 2023. Doctor tried to send him home on April 2nd, 2023.
Started on the Fourth Floor, then the 3rd, Floor, then the 2nd Floor.
Dr. Averett was the attending Doctor and was with him to the Fourth Floor where they admitted him. His rating (the doctor) was a one star.
Blood work bad = Lactic Acidulous (sp) was off never once stated that he had infection. His magnesium was low.
Never diagnosed infection yet clearly he had infection! Rapid heart rate and high blood paressure.
He coded the machine said stroke, they put respiratory instead.
After the other doctor Hospitalist took over Angela (the doctor) and me argued over antibiotics, she apologized for missing the white count and started antibiotics in which he started improving tremendously.
He left Trident on April 28th, he was there for 28 days. From April 28th, 2023 to Oct. 22, 2024 is when he passed.

We intend to pursue legal action against Trident for gross negligence and medical malpractice resulting in severe harm and eventual death of our loved one. The hospital's reckless actions and failure to provide proper medical care directly contributed to his suffering and irreversible decline.
